SUBJECT: Fulltime PHYSICAL THERAPIST

SPECIFIC POSITION SUMMARY

To evaluate, plan, develop and implement physical therapy services in accordance with current federal, state and local standards governing the sub acute, outpatient, and long term care settings to ensure that the highest degree of quality care is maintained at all times. Responsible for all activities related to patient care, interdepartmental communication as appropriate, and other duties as assigned by rehabilitation supervision.

ESSENTIAL FUNCTIONS OF POSITION

1. Evaluate, plan, develop, organize and implement resident care programs through the utilization of selected physical therapy procedures including but not limited to outcome measures, therapeutic exercise, gait training and functional training.

2. Applies selected muscle, nerve, joint and functional ability tests to assist physicians and other health personnel in determining diagnosis, prognosis and resident progress.

3. Evaluates, fits and adjusts prosthetic and/or Orthotic devices and recommends modifications to orthotics or prosthetics.

4. Participates in conferences, consultations, Facility Care Plan meetings (as needed) and educational programs designed to improve resident care and staff performance.

5. Participate in Rehabilitation Team Home Evaluation visits as needed.

6. Instructs the family and the resident in physical therapy procedures to be continued at home.

7. Consults with physicians concerning goals and treatment plans and monitors the presence of any contradictions to treatment.

8. Ensures all physical therapy documentation conforms to documentation/billing policies and procedures and meets requirements for third party payer documentation and reimbursement.

9. Conducts routine quality assurance studies and program evaluation indicators.

10. Provides direction and assistance to instruct, supervise, and coordinate the individual assignments of physical therapy assistants, Techs and students.

11. Assures that staff (PRS and Facility) receives regular, periodic training in safety procedures, treatment and carryover techniques and strategies.

12. Assists in supervision and education of affiliating students, under direction of their clinical instructor.

13. Assures the safety of patients, families and employees to include ensuring that the environment is maintained in a sanitary, attractive and orderly condition at all times; regularly monitors the physical plant and equipment to inspect, report and/or correct any situations or conditions that may cause safety hazards.

POSITION REQUIREMENTS

Education: Graduate of an approved school of physical therapy and licensed in the state of NJ.

Experience: Prior experience in SNF & long-term care setting preferred.

Knowledge/Skills: Knowledge of the federal, state and local regulations that affect physical therapy and rehabilitative care delivery. PT evaluation should include but not limited to graded therapeutic exercise, gait & posture training, general conditioning, ADL training, modalities, massage, home visits, decubitus care, balance training, edema control, joint mobilization, myofascial release, neuromuscular facilitation, scar management, soft-tissue mobilization, stretching, instruction in body mechanics, and wound care. Ability to work effectively with all persons and disciplines. Must be of good moral character and have sincere desire to work with patients, staff (facility and rehab department), visitors and family members.

PHYSICAL AND SENSORY DEMANDS OF THE JOB WITH OR WITHOUT THE AID OF MECHANICAL DEVICES INCLUDED BUT NOT LIMITED TO

Stooping, kneeling, crouching, crawling, reaching, handling, feeling, frequent lifting with mechanical aides 21 to 50 pounds, grasping, fine hand coordination, ability to read, understand and write in English, ability to communicate with personnel and clients, ability to evaluate and interpret information and make independent decisions, ability to hear, and ability to remain calm under stress.

Job Type: Full-time

Pay: $80,000.00 - $99,000.00 per year
