Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls – enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home.
We offer an array of medical and chronic care services and we’re backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years.
Homebase Medical is expanding rapidly into the Upland, Glendora and Pomona, CA areas. We are currently searching for both entry level or experienced full-time or part-time Advanced Practice Clinicians (Physician Assistant or Nurse Practitioner) to join our expanding house call practice.
Responsibilities include:
- Perform primary care in the home setting, Board and Care, and Assisted Living Facilities in the Upland, Glendora and Pomona, CA areas and surrounding communities.
- Drive to patient locations in your assigned geographic area.
- Average 7-10 patient visits per day
- Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
- Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
- Provide referrals to appropriate medical resources.
- Utilize our EMR system to document patient visit/interaction accurately and timely.
- Knowledge of HCC coding.
- Active participation in an on-call rotation is required.
- Other duties as assigned.
Qualifications and experience:
- Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
- Current, unrestricted PA or NP license to practice in California including DEA license.
- Minimum of 1 year of experience in primary care, hospice, or urgent care/outpatient setting.
- Previous experience in a home health, hospice or other home visit setting is highly desired.
- Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding
- Strong computer skills including MS Office and Outlook is strongly desired.
- Great verbal and written communication skills
- Current drivers’ license, auto insurance and reliable transportation
- Spanish speakers are strongly preferred.

What's in it for you?
- Pay scale information: $100,000 to $212,000 is the expected total annual earnings after factoring in all forms of compensation including salary, fee-for-services (FFS), benefits (vacation and sick), etc. The base salary / salary range for this position is $64,480. In addition to the base salary, the Fee-For-Services (FFS) compensation, calculated based on the number of services performed or procedures completed, ranges from $35,520 to $147,520 per year.
