Summary:
Essential Duties and Responsibilities:
- Interviews patient and family members; prepares psychosocial assessments, addressing all pertinent issues of participants.
- Evaluates data and work with a care team to develop a single plan of care that supports the patient's overall health and well-being.
- Educates and assists patient/family in understanding and adjusting to the diagnosis/disability, treatment plan, and medical recommendations.
- Provides age-appropriate and culturally sensitive counseling and individual/family treatment as well as assistance in family life crisis situations.
- Prepares and maintains all pertinent records, statistics, and progress notes.
- Assists in the development of various program curricula.
- Provides day-to-day supervision hours for MSW interns at the assigned locations (as needed) Works with ChapCare’s Care Coordinators to facilitate referrals for patients to appropriate specialty services (psychiatry, etc) and community agencies (for social services) as appropriate.
- Works with outside agencies to ensure coordinated care for victims of domestic violence, etc.
- Works with medical providers for an integrated approach to care during warm hand-off
- Performs all other related duties as appropriate and/or assigned.
Experience & Education:
- Masters in Social Work minimum requirement.
- Licensure as a Licensed Clinical Social Worker by the California Board of Behavioral Sciences minimum requirement.
- Minimum 2 -4 years experience.
Skills:
- Counseling, listening skills.
- Ability to work with an entire lifespan and diverse population.
- Bilingual English/Spanish (desired).
- Skills and Abilities vested due to education, training, and licensure as L.C.S.W.
