Job Location:

This position will be domiciled at the Rolla Satellite office. Will have the ability to work remotely a portion of the week upon successful completion of the initial onboarding period.

Role Description

This is professional and administrative work involving the evaluation and eligibility of applicants and recipients of services in a comprehensive purchase-of-service and community placement program for person with developmental disabilities.

What you’ll do

  • Accept referrals for services from individuals, family members, agencies, and other interested parties.
  • Communicate with families about obtaining services by explaining services and eligibility, to assist in completing the application materials. Screen individuals to see if DMH-DD is the most appropriate service agency to meet their needs.
  • Reviews application for services.
  • Gather review and evaluate collateral information such as medical, educational and psychological records from a variety of sources in a timely fashion; identifies and correctly codes diagnoses.
  • Administers, scores, and interprets assessment tools including but not limited to the Missouri Adaptive Ability Scale (MAAS), Vineland Adaptive Behavior Scales (II).
  • Coordinate the initial MAAS with the Division Assessment Team.
  • Determine initial eligibility for persons applying for services based upon specific guidelines as set forth in State regulations.
  • Accurately and timely complete required documentation of eligibility/ineligibility findings and required notifications to applicants according to specific timelines.
  • Enter individual information into an electronic case record.
