What we’re looking for in you

Required:

• Licensure from an accredited educational program at a bachelor's level (BSW) or master's degree (LGSW, LISW, LICSW) level
• Must have Minnesota (MN) or Wisconsin (WI) Social Work license upon start date (If licensed in only one state upon hire, must be willing to obtain license in the other state and provide prook of license within 4 months of start.)
• MN must have one of the following:
  • Licensed Graduate Social Worker (LGSW) or
  • Licensed Independent Social Worker (LISW) or
  • Licensed Independent Clinical Social Worker (LICSW)
• WI must have one of the following:
  • Advanced Practice Social Worker or
  • Licensed Clinical Social Worker
• Must have vehicle, driver's license and current auto insurance. Must meet all criteria required under Lakeview's Automobile Liability Guidelines.

The skills/experience you bring as a Hospice Social Worker

Preferred:

• Homecare or Hospice experience

The purpose of the Hospice Social Worker role

The Hospice Social Worker provides supportive services to help meet the patient/family social, emotional, economic, and environment needs in relation to the impact of a chronic, acute, or terminal illness through education, support/counseling, and referrals/coordination of community resources. This position also provides patients/families with the psychosocial support needed to cope with chronic, acute, or terminal illness through education, support/counseling, and referrals for community resources. In addition to conducting community visits, some work may be required in palliative care and working directly with the hospitalist team performing program development and inpatient consults at Lakeview Hospital.
